Collect the info that actually matters

Your clients can complete customized digital intake forms before their first session. You can easily access their responses anytime from your dashboard. This guide shows you how it works.

      Overview of Intake Forms in Allia

      Allia helps you streamline your intakes with customizable intake forms that help you gather the information you need to know and begin building an alliance with your clients from the time they sign up.

      Creating and Customizing Forms

      To access the Intake Form feature, navigate to the “Management” page in the Allia platform. Then click on the “Intake” tabTo add questions to the form, click the “Add Question” button.

      You can select from the following question types:

      • Short free response (<150 characters) - ex: chief complaint

      • Long free response (<350 characters) - ex: describing past treatment

      • Multiple choice/single selection - ex: access to firearms

      • Multiple choice/multiple selection - ex: symptom checklists

      • Numerical Scale with min/max constraints - ex: rating average level of pain or distress

      Questions are optional by default. Turn the “Required” toggle on to make a question mandatory

      Delete unwanted questions by clicking the trashcan icon.

      You can return to this page at any time to edit your intake form.

      Click “Save Form” to save any changes when you are done.

      Obtaining and Reviewing Intake Information

      Your clients will complete the intake form the first time they use the patient app (part of the onboarding process)

      You can review your client’s answers to intake questions by clicking the “intake” button in their dashboard page.
